All attendees who do not already have base access (active DOD ID card or DBIDS) must submit a base clearance form.

Forms must be legible, and must include both your social security number and another form of identification with the expiration date. Leave block 24 blank.
Submit forms and questions to Sue Moore by October 15.
Each Sponsor Table includes tickets for 8 guests. Four additional guests of the NPS Foundation will join your table for a total of 12.
